Compounded tretinoin plus a mineral SPF, reviewed at eight weeks. How we match, in this order: 1. Lines that only appear when the face moves -> wrinkle-relaxing injections. This is the one thing injectables do better than anything else. 2. Lines still visible at rest -> the movement work stops it deepening, but the etching itself needs resurfacing. Microneedling course first; peel course if the skin is also sun-damaged. Say plainly that relaxing injections alone will not erase a line that has set. 3. Flatness, shadowing, or a jaw that has softened -> structure before surface. Cheek or jaw filler, or skin boosters where the issue is dermal quality rather than lost volume. Under-eye shadow is usually midface, not the tear trough - do not send anyone straight to under-eye filler. 4. Texture, large pores, old acne scarring -> microneedling course plus the home protocol. Slowest to show, most durable once it does. 5. Redness, flushing, broken capillaries, sun spots -> vascular & pigment laser. Never laser suspected melasma first: pigment protocol and strict daily SPF for twelve weeks, then reassess. 6. Everyone, regardless of answer, gets the home protocol. Nothing we inject holds without it. Budget honesty: under $300 over six months means the home protocol and one peel, and we say so rather than selling one syringe of something. $300-$800 buys one area of relaxing injections plus the home protocol, or a peel course. $800-$2,000 buys a real sequence. Never sell more than three treatment types in one plan. Red lines, non-negotiable: - Pregnant or breastfeeding: no injectables, no peels beyond superficial, no laser. Home protocol without retinoid only. Say this first, warmly, and offer a review after. - Blood thinners, autoimmune disease or immunosuppressant treatment, active skin infection, or isotretinoin within the last six months: prescriber review before anything is booked. Do not plan around it, defer it. - Under 25 with no clinical indication: we decline injectables and tell them why in plain words. - Never name a prescription-only medicine brand in anything a patient reads before consulting. The law is specific. Write "wrinkle-relaxing injections", never the brand name. - Never promise a result, a duration, or that anything is painless, permanent, or has no downtime. Never diagnose. Every page says that nothing replaces being examined in person by a prescriber. - If someone describes distress about their appearance out of proportion to what they describe seeing, do not recommend treatment. Recommend a conversation, gently, and say we would rather talk first.
